This can only be completed if you are challenging the Penalty Charge Notice within 14 days of the issue date.
If you have already had an initial challenge rejected, you must either pay the Penalty Charge Notice or await the Notice to Owner.
If you have received a Notice to Owner, this must be completed and sent to the address shown or the appropriate payment made.
